Staff Applicant Forms

All staff applicants must arrange for at least three Blue Star reference forms to be completed and returned to us on your behalf and must also fill out and return the background check authorization form to the Blue Star offices. We require that all of these forms be returned to us prior to scheduling an interview.

Please note that your references will play a vital role in our employment decision-making. We prefer that your references be from former and current employers as well as those persons who can attest to your experience working with children.

Please choose your reference providers carefully and remember that references from family members or friends will not be honored. When we receive your reference forms and your complete application, we will contact you to schedule an interview.
